Job Description Summary
The Associate Medical Director assists the Medical Director in the overall management of medical care of all patients for Hospice of Holland, Inc. Collaborates within the medical community to increase awareness of and referrals to hospice care. Provides presentations for Hospice Orientation Program and other continuing education programs as appropriate. Available for community presentations as needed.
Essential Job Functions
- Provides medical consultation to all Hospice personnel of the various programs in Hospice of Holland. Assumes the responsibility of primary physician for patients admitted to Hospice without a qualified primary care physician.
- Works as a member of the leadership team to develop and achieve the strategic plan while continuing to identify opportunities for improvement.
- Participates in admission and re-certification process and in the weekly interdisciplinary team meetings.
- Makes visits to patients as appropriate i.e. acuity, date of admission, pain and symptom management, etc.
- Provides consultation as needed with any patient’s attending physicians regarding symptom control and/or other clinical care plan issues. Serves as a consultant to community physicians regarding potential Hospice and/or palliative care patients.
- Oversees the medical management of the Palliative Care program, collaborating on standing protocols, practice standards and outcome measurements.
- Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course, as appropriate to ensure cost-effectiveness and Medicare compliance.
- Develops and maintains effective and positive relationships with health care professionals within the community. Serves as a source of end-of-life medical expertise, educating, and promoting the benefits of hospice care.
- Available twenty-four hours/day, seven days/week for emergency medical consultation with Hospice personnel to discuss any aspect of the plan of care and identifies alternate coverage when unavailable.
- Actively participates as a member of the Professional Advisory Committee, the Quality Assessment and Performance Improvement Team. Reviews and approves policies and procedures related to medical management.
- Understands and assures compliance of regulations and standards particular to state, federal, and JCAHO accreditation program pertinent to patient care.
- Manages the activities of the Associate Medical Director(s) and mid-level providers.

Education and Experience:
- Possesses current license, (M.D. or D.O.) to practice medicine in the state of Michigan.
- Has board eligibility/certification in internal medicine, family medicine, oncology, emergency medicine, or hospice and palliative medicine.
- Is board certified in palliative medicine or will become board certified in palliative medicine within 2 years of hire.
- Has practiced medicine at least three years.
Skills and Knowledge:
- Understands the philosophical and technical aspects of hospice care.
- Demonstrates a knowledge base in technology of palliative medical management especially pain and symptom management.
- Medicare and Medicaid eligible.
- On staff at Holland Community Hospital and Zeeland Community Hospital and other hospitals as required to service patients in Hospice of Holland’s service area.
- Demonstrates excellent communication skills and experience with conflict resolution.
- Practices principles of quality management and ethical issues in Hospice care.

Works under the general guidance of the Medical Director. Normally receives little instruction or control on day-to-day work. Plans the sequence of detailed steps to accomplish the work using experience and education and solves, through initiative, most problems of detail that come up in the course of work. Interprets policies and guidelines and applies them to routine situations that arise. Personally responsible for accomplishing prescribed results and objectives, and has considerable freedom while the work is in progress. Provides guidance to clinical staff.

While performing the duties of this job, the employee:
- Is subject to varying and unpredictable situations. Handles emergency or crisis situations.
- Must drive in various weather conditions on roads in varying degrees of repair. Travel required.
- Works within an office setting, but is frequently required to visit patients in their home and in facilities.
- Is required to be on-call periodically to include evenings and weekends.
Continuing Education Requirements:
Hospice personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, Hospice personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. Must complete appropriate Hospice Orientation Training (HOP) sessions as determined by their supervisor.
